Here is an easy plan, something for the substitute file or for one of those days when you have a headache, too much stress, etc. This is for upper grades, and it is a good review of patriotic songs. The only prep on your part, after you decide on what to use, is to type, print, and copy the quiz form.
Choose four or five standard patriotic songs the kids know very well and have sung a lot. For example, "America," "America The Beautiful," "Star-Spangled Banner," "This Land Is Your Land," "God Bless America," "Grand Old Flag," etc. List them at the top of the page, denoting them as A, B, C, etc. Down below, in scrambled order, print and number about 20 phrases (first verse only unless your kids are super-smart and have the subsequent verses memorized) from any of the listed songs. It's best to use phrases from later in the song instead of the dead give-away first phrase. In front of each numbered phrase, place a blank so they may put the letter for which of the listed songs this phrase comes from! It sounds easy but it was not, and my kids were very well-versed in their patriotic songs.
Example:

- _____ 2. For purple mountain majesties
- _____ 3. From California to the New York island
- _____ 4. You're the emblem of the land I love
and so on. Grade if you wish.
